Z-Plasty Scar Revision (Cheek to Upper Lip, Post-Trauma)

Scar — Pongsak Clinic, Hua Hin Road 55

The patient presented with a long diagonal post-traumatic scar running from the lateral cheek across the upper-lip junction, healed thick and pink in the years following the original injury, its straight line conspicuous against the natural curves of the face. Treatment was Z-plasty scar revision under local anaesthesia: the original scar was incised, then two triangular flaps were raised and transposed in a Z-pattern to reorient the scar segments along the natural skin tension lines, breaking the continuous straight line into shorter, less perceptible angles. Closure was completed with fine sutures. The follow-up photograph shows the scar replaced by a finer, broken-line pattern that blends into the cheek's natural contours.
