Helical Rim Keloid Excision (Upper Helix)

The patient presented with a sizeable lobulated keloid arising from the posterior surface of the upper helix, projecting outward by roughly 1.5 cm. The lesion sat at the rim of the auricle and was visible at conversational distance from any side view. Treatment consisted of complete intralesional excision under local anaesthesia, with the wound closed in layers along the natural curvature of the helix to minimise tension on the new scar. The follow-up image shows a thin, well-aligned linear scar conforming to the helical rim, with the auricular contour fully restored and no recurrence at the time of review.
