Philtrum Scar Revision (Post Cleft-Lip Repair)

Scar — Pongsak Clinic, Hua Hin

The patient presented with a residual philtrum scar following a previous cleft-lip repair. The scar ran vertically through the right philtrum column, was wider than the surrounding skin, and slightly displaced the vermilion (lip-edge) line. Treatment consisted of a scar revision: excision of the old scar tissue, careful realignment of the orbicularis oris muscle and vermilion border, and layered closure to reconstruct a finer philtrum column. Following complete healing, the scar is significantly less visible, the right philtrum is more symmetric with the left, and the lip line is realigned.
